What Is an AV Graft?

An AV graft is another type of dialysis access. Grafts are used when a fistula is not feasible due to vein size, anatomy, prior failed fistulas, or time urgency. During the procedure, a surgeon places a soft, flexible tube under the skin and connects it between an artery and a vein. This tube creates a reliable pathway for blood to flow from the artery through the graft and into the vein, providing a suitable area for hemodialysis needles during treatment.

How an AV Graft Works

The graft acts as a bridge between the artery and vein, allowing strong blood flow for hemodialysis. Because the graft material is durable, it can be used repeatedly during dialysis sessions.

Benefits of an AV Graft

Although AV grafts have slightly higher complication rates than fistulas, they offer important advantages over other temporary access types, such as a Central Venous Catheter (CVC).

  • Ready for dialysis sooner than a fistula, often within 2 to 4 weeks – and some early-cannulation grafts can be used in 24-72 hours
  • Fully under the skin, which reduces infection risk compared to a catheter
  • Provides more reliable, stable access than a central venous catheter
  • Made of durable material that can withstand repeated needle use
  • Can offer long-term use with proper care and monitoring

Caring for an AV Graft

AV grafts require the same daily care as fistulas. Patients are encouraged to check the graft every day by feeling for the vibration known as the thrill. The skin over the graft should be washed regularly to keep it clean for needle access. Periodic evaluation and, when needed, angioplasty may be required to keep the graft functioning well. Over time, stenosis and clotting can occur, which may require intervention or graft replacement.
